Travel, Motor Vehicle and Personal Accident claim forms are available from the Manager, Insurance and Risk Management, Kazia Lombardo [t: 6620 3696; e: firstname.lastname@example.org] .
The following procedures should be undertaken in relation to any event likely to result in a claim:
- Following an accident or loss all reasonable steps should be taken to protect any property from further damage and to minimise the loss.
- Incidents that may give rise to a claim should be reported to the Manager, Insurance and Risk Management, Kazia Lombardo [t: 6620 3696; e: email@example.com] immediately as delayed notification could result in denial of the claim by the insurers.
- Loss by theft and/or malicious damage should be initially reported to Security who will in turn inform police, however it is not Security's responsibility to complete claim forms.
- Supporting documentation such as original purchase order, invoices and replacement quotation must accompany the claim where possible. Incomplete documentation will result in delays in claim settlement.
