Unit description
Examines common chronic and complex health conditions such as asthma, diabetes, chronic obstructive pulmonary disease and cardiovascular disease including the pathophysiology of these conditions. Students will focus on a patient centred model of care that includes the interprofessional team and will demonstrate the provision of effective education and support to help patients in the self management of their conditions. This unit will include caring for chronic conditions in indigenous communities and other Cultural and Linguistically Diverse [CALD] communities.
