Student Forms at SCU
Throughout your journey at Southern Cross University, you may need to complete a form to request changes, apply for support, or manage your enrolment. Whether you're applying for special consideration, updating your details, or requesting a transcript, we've made it easy to access the forms you need.

- Special Consideration – Apply for extensions or adjustments due to unexpected circumstances.
- Change of Name – Update your legal name in university records.
- Leave of Absence – Take a break from your studies and return later.
- Unit Approval Requests – Request approval to enrol in restricted units.
- Academic Transcript and AHEGS – Order official academic records, eQuals and graduation documents.
- Student ID Card – Apply for or replace your SCU student ID card.
- Review of Exclusion – Request a review if you’ve been excluded from your course.
